Application Requirements: VSLO Application Requirements

Please see below for the list of VSLO application requirements for the 2026 Visiting Student Program year. Specific electives may have additional requirements which can be viewed on VSLO.

$150 Application Fee Receipt
CV/Resume
Liability Insurance acknowledgement
I certify that I am covered by liability insurance meeting the VP&S requirements: $1 million per occurrence; $3 million aggregate. If not, I will purchase supplemental insurance to meet this requirement.
OSHA/Infection Control Certification
Documentation of current OSHA or Infection Prevention and Control/Bloodborne Pathogens certification. [ If you need OSHA/Infection Control training, please visit: https://www.workshopsexpress.com/infection-control-training. The fee for the course is $17.00 USD, and can be paid using a credit card. ]

$278 Elective Attendance Fee Receipt
$72 SHS Fee Receipt
Completed Clerkship Requirement acknowledgment
Students must have completed core clerkships in 1) internal medicine, 2) general surgery, and two other specialties – including one in the same specialty in which they wish to take an elective – prior to the elective start. This equals a total minimum of 4 clerkships.
Step 1 Exam
Students must have passed Step 1 (or COMLEX Level 1) by elective start; please upload score report.
Liability Insurance
Please upload proof of liability insurance (COI or MOI, and supplemental insurance if required).
Host Immunization Form acknowledgment
Student Health will review completed health forms submitted through the Columbia University student health portal during onboarding once students have been accepted to an elective.
